Summer camp forms checklist
Camps vary, but the paperwork rarely does. Here's the full list of forms most summer camps ask for — day camp or sleepaway — so nothing surprises you the week before drop-off.
The core forms almost every camp requires
- Registration / enrollment form — child's details, session, and parent/guardian info.
- Health history — conditions, allergies, medications, and past illnesses.
- Immunization record — vaccine dates, or an exemption where allowed.
- Physician's physical exam — often dated within the last 12 months.
- Medication authorization — dosages and permission for staff to administer.
- Photo & media release — consent to use your child's image.
- Authorized pickup & emergency contacts — who may collect your child.
Forms some camps add
- Insurance information (carrier, policy/group number).
- Swim ability or activity waivers.
- Behavior or code-of-conduct acknowledgment.
- Transportation / bus permission.
- Dietary restriction details.
The one that trips people up: the physical
The physician's exam is the most common reason a camp packet is late. Many camps require it to be signed within 12 months of the camp start date, which usually means a doctor's appointment. Book it early — the paperwork itself takes minutes, but the appointment can take weeks to get.
