Daycare enrollment checklist

Licensed daycares and preschools ask for a consistent set of paperwork before a child can start. Here's the full list, so you can gather it in one pass instead of chasing one missing form at a time.

Schedule the physical first

The physician's health assessment is the single most common reason enrollment stalls — it needs a doctor's appointment, and those book out. Get it on the calendar as soon as you know your start date; everything else on this list you can complete at home in an evening.

Fill the whole packet in one sitting

Nearly every form here asks for the same facts — birthdate, immunization dates, allergies, physician, insurance, emergency contacts. Save them once in Cubby and it fills each daycare's forms automatically; you review and sign. Your child's information stays encrypted on your own device and is only sent when you approve it.
